      • A Practical Guide to the Regulatory Reform (Fire Safety) Order 2005

        02
Fire safety has been at the forefront of the public consciousness since the tragedy at Grenfell Tower in 2017. The Regulatory Reform (Fire Safety) Order 2005 is the main piece of legislation dealing with fire safety in premises other than dwellings in England and Wales. It imposes numerous duties on employers, landlords, occupiers which are complex and often confusing.

This book provides a simple overview and explanation of the Order and the legal duties imposed by it for non-lawyers and lawyers alike. It explains who is responsible for complying with those duties and to what extent, how those duties are enforced through investigation, formal notices and prosecution and the potential consequences of breach.

        ABOUT THE AUTHOR

Laura Phillips is a specialist regulatory barrister at Six Pump Court Chambers.

She advises and acts for individuals, companies, and regulators and accepts instructions on a direct access basis.

Her practice encompasses all aspects of regulatory law including fire safety, health and safety consumer protection, food hygiene/food safety law and environmental law. She is ranked in Chambers and Partners 2019 (London Bar) as an ‘Up and Coming’ Junior in Consumer Law.

      • Foundations of Safety Science: A Century of Understanding Accidents and Disasters

How are today’s ‘hearts and minds’ programs linked to a late-19th century definition of human factors as people’s moral and mental deficits? What do Heinrich’s ‘unsafe acts’ from the 1930’s have in common with the Swiss cheese model of the early 1990’s? Why was the reinvention of human factors in the 1940’s such an important event in the development of safety thinking? What makes many of our current systems so complex and impervious to Tayloristic safety interventions? ‘Foundations of Safety Science’ covers the origins of major schools of safety thinking, and traces the heritage and interlinkages of the ideas that make up safety science today.

        Features

          • Offers a comprehensive overview of the theoretical foundations of safety science
          • Provides balanced treatment of approaches since the early 20th century, showing interlinkages and cross-connections
          • Includes an overview and key points at the beginning of each chapter and study questions at the end to support teaching use
          • Uses an accessible style, using technical language where necessary
          • Concentrates on the philosophical and historical traditions and assumptions that underlie all safety approaches

        • When I Were A Lad…: Snapshots From A Time That Health & Safety Forgot

          08

When I Were a Lad… looks at the glorious-yet-risky childhoods of yesteryear before the Health and Safety officers told us we couldn’t do everything because it was too dangerous.

The sleeper hit of Christmas 2009 – more than 4,000 copies sold through Bookscan.

A book to send shivers down the spine of any parent who has spent years protecting their children from the slightest bump and bash.

A glorious romp though some of the most reckless photos from the 1930s, 40s, 50s and 60s, tagged with side-splitting captions.

Ah, the past. A time when children could play in the snow without a helmet, crampons and a risk assessment report. When footballs were made from rhino hide and cricket was played with one pad, if you were lucky. When I Were a Lad… looks at the glorious-yet-risky childhoods of yesteryear before the Health and Safety officers told us we couldn’t do everything because it was too dangerous. It reflects on a time when children were allowed in with the animals at London Zoo; a time before the car seatbelt was invented (let alone used); a time when you were allowed to dress up endangered species in goalkeeping kit and take penalties against them. The authors have trawled through the major historic archives to find some glorious photo opportunities where the safety angle of the participants was the last thing anyone thought of. Children perch happily on lethal, limb-mangling machinery, stand all-smiles on live crocodiles, feed brown bears with their hands and get scooped from the street by passing tram conductors! These truly were the days that Health and Safety forgot, back when I were a lad…

        • Safety-I and Safety-II: The Past and Future of Safety Management

          08
Safety has traditionally been defined as a condition where the number of adverse outcomes was as low as possible (Safety-I). From a Safety-I perspective, the purpose of safety management is to make sure that the number of accidents and incidents is kept as low as possible, or as low as is reasonably practicable. This means that safety management must start from the manifestations of the absence of safety and that – paradoxically – safety is measured by counting the number of cases where it fails rather than by the number of cases where it succeeds. This unavoidably leads to a reactive approach based on responding to what goes wrong or what is identified as a risk – as something that could go wrong. Focusing on what goes right, rather than on what goes wrong, changes the definition of safety from ‘avoiding that something goes wrong’ to ‘ensuring that everything goes right’. More precisely, Safety-II is the ability to succeed under varying conditions, so that the number of intended and acceptable outcomes is as high as possible. From a Safety-II perspective, the purpose of safety management is to ensure that as much as possible goes right, in the sense that everyday work achieves its objectives. This means that safety is managed by what it achieves (successes, things that go right), and that likewise it is measured by counting the number of cases where things go right. In order to do this, safety management cannot only be reactive, it must also be proactive. But it must be proactive with regard to how actions succeed, to everyday acceptable performance, rather than with regard to how they can fail, as traditional risk analysis does. This book analyses and explains the principles behind both approaches and uses this to consider the past and future of safety management practices. The analysis makes use of common examples and cases from domains such as aviation, nuclear power production, process management and health care. The final chapters explain the theoretical and practical consequences of the new perspective on the level of day-to-day operations as well as on the level of strategic management (safety culture). Safety-I and Safety-II is written for all professionals responsible for their organisation’s safety, from strategic planning on the executive level to day-to-day operations in the field. It presents the detailed and tested arguments for a transformation from protective to productive safety management.

        • For the Safety of All: A Story of Scotland’s Lighthouses

          01

Lighthouses punctuate Scotland’s coastline – a stoic presence on the edge of the landscape. Since the earliest of these hardy structures were raised, they have been a lifeline for seafarers at the mercy of treacherous weather and uncertain navigation. Today over 100 of Scotland’s lighthouses are listed buildings.

The lighthouse is now one of many maritime resources which act ‘for the safety of all’. But we are still drawn to the solitary life of the keeper, the beauty of the lens of the lamp and the calm reassurance of a flashing light on a distant shore.

Donald S Murray explores Scotland’s lighthouses through history, storytelling and the voices of the lightkeepers. From ancient beacons to the work of the Stevensons and the Northern Lighthouse Board, and from wartime strife to automation and preservation, the lighthouses stand as a testament to the nation’s innate connection to the sea.

Published in partnership between Historic Environment Scotland and the Northern Lighthouse Board.
